FREEDOM OF CHOICE PLAN

Freedom of Choice Dental Membership is an annual reduced fee dental plan designed with you in mind allowing you to receive the premiere dental care you deserve at prices you can afford! With Discounts up to 20% and a FREE annual examination and Bite Wing X-rays, you can't afford NOT to join. This plan offers the economy of group dental care but with the individualized attention of private care.

Benefits begin immediately. Members must remain in the plan a minimum of 12 months.

1 Free Periodic Exam and 4 Bitewing X-rays per year
20% Off all Dental Services *Exclusions apply
No year maximums
No Deductibles

Plan Fees:

$150 per individual patient

Patient Payments

All payments are made directly to the dental oofice. Payment in full is expected at each appointments as treatment is performed. You should discuss all the future payments and costs before new appointments are made.

Exclusions

Orthodontics, Snoring/Sleep Apnea Appliances and TMD Treatments are NOT covered by this plan.
Already Discounted Services also are NOT included in this plan. Ask for details
When Care Credit aor any other outside financing is used, the amount of the discount given on the treatment is reduced to 5%
CAN NOT be used in conjunction with dental insurance
CAN NOT be combined with any other promotional offers or discounts
